On May 11, a 28-year-old driver crashed into the back of a fire truck in South Jordan, Utah, according to the report.
Despite Tesla's Autopilot feature indicating a driver must be attentive at all times, the driver was looking at her phone and did not brake or take action to avoid the collision, according the South Jordan Police Department and the report.
Tesla chief Elon Musk said on Monday the company was going through "thorough reorganization" as it handles production problems, senior staff departures and recent crashes involving its electric cars, reported Reuters.
